I have a D200 and a 18 - 200 VR zoom lens that is working pretty good for me. I would like a longer lens though and was thinking of the 200 - 400 VR lens, but giving the $5K price tag I was wondering what I will be missing if I buy now rather than wait a year or so for the next lens to appear. What would the next step in lens production be?

No one can answer your question as to when lenses are designed. But I can tell you that the 200-400 is a special purpose lens and if you can't generate enough income from it you should be looking at something in the 75-300 range.
I don't know if generating income is a criteria for you or not. Not all of us are professionals, some are just avid photographers. You have to ask yourself if you're going to get $5000.00 worth of use out of it. I too have the D200 with the 18-200mm VR lens. I like it very much. Instead of the 200-400mm, I purchased the 80-400mm VR and it suits me just fine for my use. I guess it just comes down to if you can justify the cost or not. For me I couldn't. Best of luck.
